Yes, you can request appointments online. Please be aware that new or initial appointments are not approved until all intake paperwork has been completed.

Yes, Currently, we offer telehealth only appointments. In the near future, we will also offer in-person office visits for patients who prefer a face-to-face experience.

Unfortunately, due to liability reasons, we DO NOT write or provide ESA letters or letters for service animals. For more information on Emotional Support Animals or Service Animals, please visit https://www.ada.gov/topics/service-animals/.

Unfortunately, We DO NOT complete Social Security Disability or Long-Term Disability forms. We may complete some short-term disability forms on a case-by-case basis, and that decision is at the discretion of the mental health provider. We are unable to complete disability forms at the first visit.

Unfortunately, we DO NOT complete court/legal documents or assessments. For any civil or criminal matter, we recommend you seek the services of a Forensic psychologist who provides professional psychological expertise within the judicial and legal systems.

Yes, we will assist with school accommodation and course withdrawal forms. These letters/forms are completed by the provider outside of office hours. A nominal fee is associated with the administrative work and time required to complete these documents, which is not covered by insurance. Letter/documentation completion can take up to 5-7 days. We are unable complete school forms or letters at the first visit.
